I agree with you that's not elegant. But it works. Small files tend to be well within size of extent. And they contribute the most to overhead just because they are small and you can fit a lot of them onto a filesystem of a size. And what you've described is the worst case. There are not that many files on the border of one extent. Usually files that benefit the most from huge pages are at least several extents in size -- media, databases, etc. And ratio of "allocation of huge pages to wrong extents" diminish as file grows.
