Alexey, do you have anything approaching reliable reproduction case? What you
stated in comment #6 implied that you could reliably reproduce this. If you
do, can you give me details about the hardware and kernel versions that were
used, and the dmesg outputs, and whether LVM and/or device-mapper was involved?
