it is part of the Linux Hotplug utilities (linux-hotplug.sf.net) and
must be put under /etc/hotplug. However using Google to find it outside
the Linux Hotplug packages is also very easy. Here is the CVS version:

http://cvs.sourceforge.net/viewcvs.py/linux-hotplug/admin/etc/hotplug/firmware.agent
