Cleanup the redundant WARN_ON_ONCE(cond) + pr_warn(msg) into
WARN_ONCE(cond, msg). Also add some WARN_ONCE() for hitcount check.
These WARN_ONCE() errors makes it easy to handle errors from ktest.
