A week or two ago I saw a panic on boot in try_to_wake_up, but it was not
reproducible and I had not written down any trace information. This
evening I saw it twice more, but then on the third boot things worked fine.

The first time it happened, I remember checking the git logs and it was

sched: Update the clock of runqueue select_task_rq() selected
In try_to_wake_up(), we update the runqueue clock, but
select_task_rq() may select a different runqueue than the one we
updated, leaving the new runqueue's clock stale for a bit.
This patch cures occasional huge latencies reported by latencytop
when coming out of idle on a mostly idle NO_HZ box.

I thought this problem had solved itself, but I've hit it again three times
in the last few days.
I've expanded the CC list a little (based on get_maintainer for
kernel/softirq.c, since that seems to be involved somehow, and Rafael since
this definitely seems to be a regression), to see if anyone else has any
ideas.

I may have found the bad thing that could have happened to ksoftirqd.
If you feel like testing, try the below. We were altering the task
struct outside of locks, which is not interrupt etc safe. It cures a
problem I ran into, and will hopefully cure yours as well.
sched: fix runqueue locking buglet.
Calling set_task_cpu() with the runqueue unlocked is unsafe. Add cpu_rq_lock()
locking primitive, and lock the runqueue. Also, update rq->clock before calling
set_task_cpu(), as it could be stale.
Running netperf UDP_STREAM with two pinned tasks with tip 1b9508f applied emitted
the thoroughly unbelievable result that ratelimiting newidle could produce twice
the throughput of the virgin kernel. Reverting to locking the runqueue prior to
runqueue selection restored benchmarking sanity, as did this patchlet.

It did end up applying, but did not have any effect. Looking at the patch
again, I see that it appears to only affect CONFIG_SMP, which I am not
running (and in fact it adds a build warning for the !SMP case). So there
was not much chance of it fixing anything, I suppose.
Any other ideas? I don't have a serial console, and the trace scrolls off
my console, so I don't know if any debug printks would help. Would it help
if I copied the entire panic message entirely, including the Code section?
I can try that the next time it happens.

Use vga=ask boot_delay=100 select the highest res possible.
Possibly you could use a digital (video) camera to record the output.

FWIW, I see a crash in autoremove_wake_function() during resume from suspend
to RAM that happens immediately before the thawing of tasks. It always happens
in the "events" thread. I can trigger it quite reliably, although not 100% of
the time.
I can only see the bottom half of the call trace, so I can't really say what
kind of a crash this is.
Also, for quite some time I've been observing a resume-from-hibernation crash
during the final switch from a VT to X that could also be explained by a
problem with "events".

For what it's worth I've seen this on BFS and assumed it was a bfs issue until
I spotted this thread so I'll tell you what I discovered when I was
investigating it, but unfortunately I did not find the root cause.
Incredibly the bug happened in try_to_wake_up where the task struct that was
in the call function (p) gets dereferenced before the rq lock is grabbed. Then
when the rq lock is attempted to be grabbed it has no p to reference.
Further investigation showed it to always be ksoftirqd spawning on bootup only
and never in any other situation. The factors that were common was that there
would always be a conditional resched that occurred and that's how it would
get lost. I tried stepping through the boot process on kvm but always came up
stumped as to how on earth it even happened. The only common variable was that
it -only- ever happened with voluntary preempt enabled, and not with full
preempt or no-preempt. cond_resched is called 2 or 3 times during the boot
sequence via might_sleep by that stage, but if I removed each might_sleep one
at a time it would just happen from a different might_sleep, suggesting we
weren't sleeping when we shouldn't. Since I'm anti-fan of voluntary preempt, I
gave up trying to find the root cause and put this nonsense workaround in
__cond_resched :

And it's still there in BFS, but it fixes the problem, in case someone wanted
to use voluntary with bfs. I've long since lost the config that caused the
problem reliably and can't guarantee that it's the same thing happening on
mainline, but figured the information might be helpful.

Our oops printing code sucks. It should mdelay after each screen or
something like that. Call trace should be in reverse order, so the
most useful parts stay on the screen...

In this particular case it should fit into the screen, but it happens during
a VT switch or something like this.
Unfortunately, this issue turns out to be quite difficult to nail down and only
triggers in specific configurations.
