Subject: [PATCH 0 of 3] x86-64: Calgary IOMMU

[Andi, Jon, sorry if you receive multiple copies of this - mercurial's
patchbomb is subtle and quick to anger]

This patchset introduces support for the Calgary IOMMU found on IBM's
high end xSeries servers. It fixes almost all of the issues Andi
raised (details in the individual patches). In particular, X works
fine now - we no longer enable translation on PHB0 by default.

Comments and testing in -mm appreciated!

swiotlb relies on the gart specific iommu_aperture variable to know if
we discovered a hardware IOMMU before swiotlb
initialization. Introduce iommu_detected to do the same thing, but in
a HW IOMMU neutral manner, in preparation for adding the Calgary HW
IOMMU.

This is patch -C1 against 2.6.16-git, unchanged from previous
version.

This patch introduces the Calgary specific bits. The implementation
draws inspiration from both PPC (which has support for the same chip
but requires firmware support which we don't have on x86-64) and
gart. Calgary is different from gart in that it support a translation
table per PHB, as opposed to the single gart aperture.

Patch is -C1, against 2.6.16-git. This patch (and the following one)
fix the majority of Andi's comments:

- don't enable isolation on PHB0 (where the graphics adapter lives) by
default in case the X server causes DMA. Specify
"calgary=translate_phb0" if you want full isolation for PHB0.
- s/PHB/host bridge/ where appropriate
- hook into CONFIG_IOMMU_DEBUG
- move valid_dma_direction check to the callers
- only panic on allocator overflow if panic_on_overflow is set
- iommu_alloc returns the DMA address including the offset
- remove useless BUG_ON's
- use CLFLUSH/wbinvd inside of mb()'s
- switch per PHB register offset lookups to use arrays
- remove useless goto
- explain usage of sysdata and why it doesn't conflict with NUMA's
usage
- use constants for magic numbers when calculating initial address to
ioremap
- use ioremap_nocache
- be quiet if no Calgary is found
- document "calgary=xxx" options in
Documentation/x86-64/boot-options.txt
- no need to check for failures from alloc_bootmem_low, it panics on
failure
- add printks when enabling translation (isolation) on a PHB

Stuff left to do:

- figure out a way to run X safely with full isolation (depends on X
drivers being used? add translations for mmap on /dev/mem?)
- fix falling back to swiotlb

x86-64: Calgary IOMMU - hook it in

This patch hooks Calgary into the build and the x86-64 IOMMU
initialization paths. It makes the following changes:

- add Kconfig entry for Calgary
- add Makefile stanza for Calgary
- try to initialize Calgary in pci_iommu_init
- parse calgary=xxx boot options in parse_cmdline_early (early because
we need them in detect_calgary(), called from setup_arch())
- add Calgary boot options documentation
- make sure PCI_UNMAP_ADDR/PCI_UNMAP_LEN are defined properly for
Calgary
- add prototypes for gart_iommu_init(), no_iommu_init() and
detect_calgary() to proto.h

This is patch -C1 against 2.6.16-git. Changes from previous version:

- remove dependency on MPSC in Kconfig
- elaborate in the Kconfig message on Calgary's isolation capabilities
- switch the initialization order: first Calgary, then gart
